What form do I need to deduct a new car sales tax?

I bought a new car last year. What tax form do I need and on what line do I write the total amount of sales tax I paid? Do I add the state and county tax together? I live in IL.

   See Publication 600. The sales tax on the car, together with the sales tax from the tax tables for your state, plus a possible adjustment for your locality, goes on line 5 of Schedule A.
